Passport and Visas Services Expedited  Infant/Child!

Requirements for an Infant/Child Passport are:

For the Passport Office(Required Documentation's are):

  1. Fill out DS-11 application form (DO NOT sign the form until the Passport Acceptance Agent instructs you to do so)

  2. Submit any of the below (Proof of Citizenship):
    1. Certified U.S. birth certificate
    2. Previous fully valid U.S. Passport
    3. Report of Birth Abroad (Form FS-240)
    4. Certification of Birth Abroad (Form FS1350)
    5. Certificate of Citizenship or Naturalization from BCIS

  3. A Delayed Birth Certificate filed more than 1 year after your birth may be acceptable if it:
    1. Listed the documentation used to create it and
    2. Signed by the attending physician or midwife or
    3. Lists an affidavit signed by the parents or
    4. Show early public records


  4. Proof of Identity (Present Evidence of Child's Relationship to Parents/Guardians (NOTE: Previous US passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s). You'll need to submit one of the following):
    1. Certified US birth certificate (with parents names)
    2. Certified Foreign Birth Certificate (with parents names & translation, if necessary)
    3. Report of Birth Abroad (Form FS-240) (with parents names)
    4. Certificate of Birth Abroad (Form DS-1350) (with parents names)
    5. Court Order Establishing Custody
    6. Court Order Establishing Guardianship
    NOTE: If the parent(s)/guardian(s) name is/are other than that on these documents, evidence of legal name change is required

  5. Provide Parental Identification
    1. Valid Drivers License (issued or renewed more than 6 month ago)
    2. Valid Official U.S. Military ID
    3. Valid U.S. Gov't ID
    4. Valid U.S. or Foreign Passport with recognizable photo
    5. Naturalization/Citizenship Certificate from BCIS with recognizable photo
    6. Alien Resident Card from BCIS

    NOTE: Your Social Security Card does NOT prove your identity.

  6.  Present Parental Application Permission Documentation
    1. Both parents (if the child is under 14) must appear together with child at the Acceptance Agency and sign or
    2. One parent appears, signs, and submit second parent's Statement of Consent: Issuance of a Passport to a Minor Under Age 14, Form DS-3053 authorizing passport issuance for the child or
    3. One parents appears, signs, and submits primary evidence of sole authority to apply (such as one of the following):

    1. Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent: or
    2. Consular Report of Birth Aboard (Form FS-240) or Certification of Birth Abroad
    3. (Form DS-1350) listing only applying parent; or
    4. Court Order granting sole custody to the applying parent (unless child's travel is restricted by that order); or
    5. Adoption decree (if applying parents is sole adopting parent); or
    6. Court order specifically permitting applying parent's or guardians travel with the child; or
    7. Judicial declaration of incompetence of non-applying parent; or
    8. Death certificate of non-applying parent


    If none of the above documentation is available, the applying parent/guardian should submit Form DS-3053: Statement of Consent: Issuance of a Passport to a Minor Under Age 14.

    NOTE: A third-party in LOCO PARENTIS applying on behalf of a minor under the age of 14 must submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport. When the statement or affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.

  7. Provide two Passport Photos
  8. Pay the Applicable Government Fee
  9. Provide of International Departure; Original flight itinerary from the airline or travel agent (Departure must be within 10 business days for those requesting a passport in less than 2 weeks)
Before going to Acceptance Agent, verify that you have collected the following document:
  •  Application Form DS-11 (both pages, if downloaded)
  •  Proof of Citizenship
  •  Proof of Identity
  •  Two Passport Photographs
  •  Travel Documentation (Travel Itinerary)
  •  Government Fee
