You
may need a New or First Passport if:
- You have
never had a passport before
- Have lost
a valid passport
- Passport
is currently expired & was issued more than 15 years ago
- Passport
is currently expired & was issued prior to your 16th birthday
or you are under age 16
- Your passport
is damaged or mutilated
- Your
name has changed since your passport was issued and you do not
have a legal document formally changing your name.
Requirements
for a New or First Passport are
For
the Passport Office(Required Documentation's are):
- 2 (two)
passport sized photos
- Proof of
US Citizenship - Original/Certified Birth Certificate or Naturalization
Certificate or a previous passport
- Proof of
Identity - Valid Driver's License (not temporary or learner's
license) or
Previous US passport or Certificate of Naturalization
or government employee photo
ID as Proof of Photo Identity
- Completed
Passport Application Form DSP-11
(Do not sign this application until
instructed by acceptance Agent)
- For those
who have lost a valid passport, you need to also complete Lost
Passport
Form DSP-64
- Proof of
International Departure - Original flight itinerary from the
airline or travel
agent (Departure must be within 10 business days for those
requesting a passport
in less than 2 weeks)
- Check/Money
Order, payable to U.S. State Department. Do Not write checks
until
instructed to do so by Acceptance Agent
Note:
All applicant 13 years or younger must appear in person at the Acceptance
Agent. Minor must be accompanied by both parent or legal guardian
with acceptable proof of photo ID.
